MDS Coordinator

Strawberry Point Lutheran Home Community is seeking an experienced MDS Coordinator to support our clinical team and help ensure accurate resident assessments and care planning. This role plays a key part in maintaining compliance with state and federal guidelines while helping our team deliver thoughtful, resident-focused care in a long-term care setting.

Position Responsibilities

• Complete and coordinate Minimum Data Set (MDS) assessments according to required timelines
• Review resident documentation to ensure accuracy and regulatory compliance
• Work closely with nursing, therapy, and interdisciplinary team members to support care planning
• Help maintain compliance with CMS, state, and facility assessment requirements
• Assist with staff guidance related to MDS processes and documentation standards
• Participate in quality assurance and performance improvement efforts

Qualifications

• Current RN or LPN license in the State of Iowa
• Previous MDS experience in a long-term care or skilled nursing environment
• Strong understanding of MDS guidelines and regulatory requirements
• Excellent communication and collaboration skills
• Strong organizational abilities and attention to detail
